Physicians' Applications
Physicians applying for employment or contractual positions at STF are to complete the Mental Health Medical Staff Application (requires Adobe Reader). Fill in all required fields; then, at the bottom of the form, click "Submit by Email" to electronically submit the application to STF's Human Resources Department.
